Please fill out the following information completely to request a Unit Visitation, and when you are completely finished, press the "SUBMIT VISITATION REQUEST" button at the bottom. If you make a mistake, please email us at unitservice@shushugah.org. This form should be completed by the Unit Leader only, or his/her designee. The deadline for all requests to be submitted is February 15. Please act quickly. You are asked to list your first three date preferences, and every effort will be made to honor these requests. You should receive a confirmation and scheduled visitation from your Chapter Unit Service Committee within 7 business days. If you do not, please contact the Lodge Unit Service Committee.
