Registration forms for lodge membership, and various lodge events are available below. In order to read and print the forms, you will need the free Adobe Acrobat Reader, available for download from the Adobe web site.

2007 Dues & Registration

• Lodge Registration for the year is being conducted by using personalized, individual registration forms and an online registration system. Each form contains all the information currently on file about each member, which members are asked to update, correct, and return. Registration forms were sent to all members who were dues-paid anytime within the last three (3) years. If you did not receive, or have misplaced your form, or have any questions regarding the registration process please write to registration@shushugah.org for assistance.
